Lincolnshire Health and Care Collaborative (LHCC) is an innovative partnership which aims to improve the health and wellbeing of everyone in in the county.
Unique in scope and scale, LHCC brings together providers from across the county’s health and care system, including acute, mental health and community trusts, primary care, social care and the voluntary sector. By combining energy, expertise and resources, and working collaboratively and creatively, we are are developing a shared vision for health and care in Lincolnshire.
Our vision is to support people to lead healthy and fulfilling lives by delivering excellence in health and care for people when they need it.
