Help us improve Palliative and End of Life Care Services

At NHS Lincolnshire, we understand how important it is to learn from the experiences of those who have sadly passed away. Whether you are a carer, family member, friend, or neighbour, we truly value your feedback. Your experiences can help us improve palliative and end-of-life care services, ensuring that your loved ones’ voices are heard.

This year marks the third time NHS Lincolnshire is running this survey. The aim is to continually assess and improve the care provided. Based on feedback from previous surveys, NHS Lincolnshire has made several improvements, including:

• Enhanced and updated information on the Palliative and End of Life Care website: www.eolc.co.uk.

• Additional training for healthcare staff specifically on palliative and end-of-life care.

• Easier access to specialist clinicians for advice and support.

What is this survey about?

We are working together with families, carers, and friends to improve the experiences of patients and those around them as they receive end-of-life care. By sharing your stories—at each stage of the journey—you will help shape better services in Lincolnshire.

We understand this may be an emotional process, and we want you to feel supported throughout. If you feel upset or distressed at any point, please feel free to pause or stop the survey. Support information is provided throughout the survey.

Please complete the survey by 30 September 2025.

Survey sections

This survey will ask about the care your loved one received in the following areas:

• Care received at home
• Urgent care provided out of hours
• Care from district and community nurses, carers, and social care
• Care from the GP or GP practice team
• Care in care homes
• Last hospital admission
• Last hospice admission
• Experiences in the last two days of life
• Circumstances surrounding your loved one’s death

You do not need to complete all sections of the survey—only those that are relevant to you and the person you are responding about. The survey will guide you through the appropriate sections based on your answers.

Confidentiality
