Population Health Management, is the generation and use of insights that allows us to move from a system designed to manage ill-health, to one that enhances population health and tackles inequalities. It is a partnership approach across the NHS and other public services which focuses on individuals’ wider determinants of health and the use of insights to improve the physical and mental health outcomes and reduce health inequalities.
