The Lincolnshire Living with Cancer programme is implementing personalised treatment, care and support for people living with cancer in the county, improving patient experience and quality of life, and helping to reduce health inequalities.

What are our priorities and what do we want to achieve?
A cancer diagnosis has a profound impact on a person and those close to them, and the effects are often experienced beyond physical health. Some people experience challenges due to the after-effects of their treatment such as mental health issues, financial hardship or transport difficulties, for example. Some people feel very isolated and lonely.
Around 37,000 people are living with cancer in Lincolnshire and everyone will experience a cancer diagnosis differently, and face different challenges. That’s why we are working to transform access to all the different types of support and services that people need before, during and after a cancer diagnosis (outside of their diagnosis and treatment which is done by medical teams in hospitals and other health settings.)
Our vision is ‘a better and sustainable future for supporting people living with cancer, involving and integrating all relevant parts of the health and social care system, using the assets we already have, supporting people in the place they would like and in the way they would like, and placing people at the centre of everything we do.’

Lincolnshire Living With & Beyond Cancer Personalisation Film
Developed by Lincolnshire’s Living with Cancer team, the film aims to help cancer patients navigate what can be a daunting and complex journey.
Transitioning from hospital care back into the community can often leave people feeling lost and bewildered as they try to adjust to life beyond cancer. It covers topics including the importance of self management; managing the transition from active hospital treatment back into the community, and where to find the right type of support.
What are we doing?
People living with and affected by cancer in Lincolnshire have told us that to improve patient experience we need to change the way things are done in Lincolnshire. We need to have:
- better information sharing between services so that people don’t have to tell their story over and over again
- joined up pathways between different parts of the integrated care system.
- integrated services so that they work as a whole
- a workforce who are supported and have access to the resources they need to support their patients
- improved communications between different services and between medical staff and patients
- easier and improved access to information, advice and support.
- more support services
- Equitable services across the county that are the right fit for communities
Who is involved?
The Living with Cancer Team does not provide a service. To bring about the changes people would like to see, we work with many different organisations right across our county: county and district councils, government departments, our hospitals, primary care, community health services, voluntary, community and faith sector partners, leisure providers, businesses and utility providers, and communities, patients and the public.
