Children and young people

The Children and Young People (CYP) is an integrated programme of work bringing together NHS organisations, Lincolnshire County Council, the provider trusts, public health, primary care and the voluntary sector.

What we are doing?

  • Transition from children’s services to adult services

    Working on developing the transition from children’s services to adult services through the formation of a system wide transition network.  We know that when young people move into adulthood it can be a very challenging time for them and their families with services delivered in different ways.

    The CYP programme has set up a system wide transition network that has developed key principles for transition that will support addressing the issues of continuity and in some cases gaps in service. Whilst this sits within the CYP programme, often the biggest changes will need to happen within adult services due to differences in the way adult services are commissioned or delivered and as such we work closely across all work programmes within the ICB.

  • Care Leaver Prescription Pre-Payment Certificate

    NHS Lincolnshire ICB will fund prepayment prescription certificates for care leavers aged 18 to 25 years who are not entitled to free prescriptions under current exemptions.

    Across Lincolnshire there is a strong commitment from agencies to deliver a consistently high-quality core offer of support for Care Leavers. This cohort of young people are known to experience additional challenge in terms of both health and well-being and financial disadvantage. Provision of free prescriptions for this vulnerable group will support both access to healthcare and a reduction in health inequalities, and reflects the goals and priorities set out by NHS Lincolnshire ICB.

  • Elective and equitable recovery for children and young people

    Elective recovery including an equitable recovery for CYP is reflected in our programmes to ensure we are sighted on areas of elective recovery waits for CYP in Lincolnshire.

    An example this year of key partnership working was in a Super Saturday clinic for CYP who were waiting to be seen by a Dermatology specialist. Feedback received from our service users and their families was very positive and we had 100% attendance due to plenty of parking and the clinic being held on a weekend day.

  • Family Support Worker Emergency Department Pilot

    Lincolnshire hosted a Family Support Worker in an Emergency Department (ED) pilot, provided by Barnardo’s and funded by NHSE. Learning from the pilot has led to increased awareness and a training needs for ED staff around the holistic needs surrounding CYP and a better understanding of community services who are well placed to offer further support.
