Help us shape Lincolnshire’s Intermediate Care Strategy 2025-2030
Lincolnshire County Council, NHS Lincolnshire Integrated Care Board, and local partners are reviewing how intermediate care is delivered across the county. We want to hear from you.
Intermediate care is short-term support that helps people:
- Recover after illness, treatment, a fall, or an accident.
- Avoid unnecessary hospital or care home admissions.
- Regain independence with the help of trained professionals.
This support is usually provided for 6-8 weeks and can take place at home, in a care home, or in a hospital setting. There are four main types:
- Reablement – Helping people rebuild skills and confidence.
- Crisis Response – Urgent short-term care to prevent hospital admission.
- Home-Based Care – Support delivered in a person’s own home.
- Bed-Based Care – Temporary care in a hospital or care home.
