GP practices
Lincolnshire has 81 GP practices, and they provide primary care services to local people.
By providing funding, strategic direction, and practical support, the ICB aims to strengthen general practice and improve the health and well-being of their populations.
What are our priorities and what do we want to achieve?
Primary Care Development and Transformation
We work closely with practices and Primary Care Networks (PCNs) to develop new models of care, enhance digital services, and expand multidisciplinary teams. Our support includes:
- Investment in additional roles through the Additional Roles Reimbursement Scheme (ARRS)
- Guidance on care navigation, personalised care, and anticipatory care
- Support for the implementation of NHS England’s Fuller Stocktake recommendations
Workforce Planning and Training
We help practices to recruit, retain, and develop a skilled workforce. This includes:
- Workforce planning tools and local workforce data insights
- Coordination of training opportunities for GPs, nurses, care coordinators, and other roles
- Support for GP Fellowship and mentorship programmes
- Collaboration with local education providers and Health Education England
Quality Improvement and Clinical Effectiveness
Our Primary Care Quality team works alongside practices to support continuous improvement in care quality, patient safety, and clinical outcomes:
- Regular practice-level quality reviews
- Implementation of quality improvement tools and frameworks
- Support with CQC preparedness and action planning
- Sharing of best practice and clinical audits
Digital Support and Innovation
We provide digital tools and services that improve patient access and streamline practice operations:
- Implementation and optimisation of online consultation platforms
- Support for GPIT infrastructure, cyber security, and clinical systems
- Help with data quality and reporting for population health management
Financial and Contractual Support
Our Primary Care Contracts team supports practices with:
- Guidance on GP contract changes and enhanced services
- Management of GP funding streams, claims, and reimbursements
- Support for new service models and commissioning arrangements
Wellbeing and Resilience
We recognise the pressures facing general practice and offer wellbeing support for practice teams, including:
- Access to occupational health services and mental wellbeing resources
- Resilience workshops and peer support networks
- Promotion of healthy working environments and flexible working options
Collaboration and Integration
We bring practices together with other health, care and voluntary sector partners to support integrated care across communities. This includes:
- Development of Neighbourhood Teams with social care, community, and voluntary services
- Support for PCNs and Integrated Neighbourhood Working
- Facilitation of shared learning and collaborative initiatives
Primary Care Networks (PCNs)
Primary Care Networks (PCNs) are groups of GP practices working together with other local organisations, such as community, mental health, social care, pharmacy, hospital and voluntary services. They will support the needs of a population that has grown, is living longer, and may need to access local health services more often.
In Lincolnshire, PCNs are based on GP registered lists and serve communities between 29,000 to 77,000. They are designed to still provide the personalised care valued by patients, but be big enough to have impact and economies of scale through closer working.
