It’s All About People: Lincolnshire’s Personalisation Programme

What is personalisation?

Imagine a world where:

  • You are the most important person in your health and care.
  • You’re seen as an equal and active partner in conversations and decision-making about your health and wellbeing.
  • You’re not just a ‘patient’ with symptoms and separate conditions. Instead, you are seen as a whole person with goals, values, lifestyle, family, and a community, because all of these play a part in your health.
  • You’re not offered a one-size-fits-all approach. Instead, you are supported to make a shared and informed decision about your health and wellbeing needs so you can live the life you want to.
  • You only have to tell your story once, and the focus of the conversation is built around the question “What matters to you?”, not “What’s the matter with you?”

This is personalisation.

What are our priorities and what do we want to achieve?

The It’s All About People Personalisation Programme Team brings together the expertise and experience of individuals drawn from the NHS in LincolnshireLincolnshire County CouncilLincolnshire Voluntary Engagement Team (LVET) and the broader Voluntary, Community, Faith and Social Enterprise Sector (VCFSE), and Experts by Experience – people who use health, care, and wellbeing services right here in Lincolnshire.

Together, this team is focused on one thing: making care truly personal. This is being achieved through:

Building and nurturing a two-way partnership

Personalised care isn’t something we do to people – it’s something we create with them.

It relies on people feeling confident to ask questions, share what’s important, and take an active role in their care. At the same time, it requires health and care professionals to listen deeply and use that information to tailor care, and support treatment in a way that feels right for the individual.

Supporting prevention and self-management

Helping people stay well is just as important as treating illness. That’s why personalised care includes supporting people to manage their own health, make positive lifestyle changes, and feel confident in taking small steps that make a big difference. It’s a vital part of prevention and of helping people enjoy the best possible health and wellbeing.

Supporting the shift to community-based health and care

When care is personalised, it’s designed around people’s lives, not just their health conditions. That means understanding their support networks, daily routines, and community connections and resources.

By tailoring care in this way, we can deliver more care in local settings, build stronger, more trusted relationships within communities, and help people stay well and independent in their own homes for longer.

Personalised care makes community-based care relevant and responsive, so it truly meets people where they are.

Tackling health inequalities

Personalised care also plays a key role in tackling health inequalities. By adapting services to meet people where they are – and understanding the barriers they face – we can help make health and care more fair, more accessible, and more inclusive.

This is how we move toward a health and care system that works better for everyone.

Changing culture

Delivering truly personalised care is not without challenges. It means shifting long-held ways of working, changing conversations, and building a new culture – something  that takes time and all of us to make happen.

But change is possible – and the It’s All About People Personalisation Programme Team is leading this change across Lincolnshire.

Personalisation is key to creating a sustainable health system in Lincolnshire that delivers for everyone 

Personalisation is a key component of both the Better Lives Lincolnshire Integrated Care Partnership (ICP) Strategy and the NHS Lincolnshire Joint Forward Plan 2023 – 2028:

Better Lives Lincolnshire ICP Strategy: 

Personalisation is one of the strategy’s five key enablers.

NHS Lincolnshire Joint Forward Plan: 

 ‘A better relationship with the public is Priority 1 and underpins all other strategy priorities.

This better relationship is being created through Our Shared Agreement, a shift to people and healthcare providers making decisions together, people and professionals co-producing improved or new services, and supporting people and their families to manage their own health and wellbeing.

What are we doing? It’s All About People Programme workstreams

The It’s All About People Team is working across the following areas:

  • Nurturing and growing the potential of people and communities
    • Nurture a better relationship between the public and health and care providers in Lincolnshire through Our Shared Agreement.

    • Support people to make decisions about their health and care with their healthcare providers, centred on ‘What matters’ to them.

    • Encourage and support people to manage their physical and mental health, remaining as independent for as long as possible within their local communities.

    • Nurture and grow strong and vibrant local community networks and services to support people.
  • Co-production and collaboration
    • Work with people with lived experience, clinicians, practitioners, VCFSE organisations, Public Health, Population Health Management, Digital, and Health Inequalities teams, to co-produce solutions and design improved and new services and approaches.

    • Create public awareness campaigns and educational materials to ensure that both health and care professionals and the general public understand personalisation and are active in making it ‘the way we do things’ in Lincolnshire.
  • Workforce development and system learning
    • Connect staff across the Lincolnshire health and care system to create opportunities to grow, learn and lead on personalised care together.

    • Providing a range of learning and development courses, events and other opportunities to improve health and care workforce knowledge, skills and confidence to deliver personalised care.
  • Driving change and innovation
    • Be ‘Change Makers’ and catalysts for change, planting seeds, making ripples, being curious, creating discussion, and challenging the thinking and ways of working of the system.

    • Identify, inspire, connect and support other change-makers within Lincolnshire, to build on the strengths of what has already been achieved and ensure the momentum for positive change continues with future generations.

    • Collect and share stories, experiences, resources, and practical solutions to challenges to drive the passion, commitment and ability to work differently together and improve outcomes for all.
  • Measuring and demonstrating the impacts of personalisation
    • Continually evidence the impacts of personalised care for people, the health and care workforce, and the wider Lincolnshire system.
