Staff Hub – Lincolnshire Care Record

The Lincolnshire Care Record is an online system that enables staff across the Integrated Care System (ICS) to view one integrated care record for their patients.

It brings together selected patient information, from multiple organisations, in real time.

In the future, patients will also be able to see their own care record here.

  • Who uses it?

    The Lincolnshire Care Record is used by all of the ICS providers, linking their patient systems to provide a single view of the patient's healthcare record.

    This diagram illustrates the network of ICS providers across Lincolnshire and neighbouring regions who actively access and utilise the Shared Care Record. The system is seamlessly integrated with each provider’s clinical systems, enabling real-time extraction of vital patient data into a unified, comprehensive view of the patient’s health record.

    This integration empowers clinicians with immediate access to rich, cross-organisational patient information—supporting safer, faster, and more informed decision-making across the entire ICS.

  • How does the Lincolnshire Care Record work?

    The LCR pull valuable patient data from across Lincs ICS as well as its neighbouring NHS providers together in one place providing health and care workers with access to rich patient information.
