How can the Lincolnshire Care Record support better care?

The Lincolnshire Care Record is made up of four separate but integrated modules. Each module offers healthcare providers and their employees with rich features and functionality designed to enhance patient care and improve collaboration between ICS providers.
Care Portal: Allows healthcare staff across the ICS to access patients and service users the shared care record. Records are updated from the providers electronic patient record systems to ensure the data is accurate and comprehensive.
Care Community: Care Community is a solution for caregivers, patients and their proxies to improve communication, close gaps in care, and improve health outcomes as well as increase care team efficiency.
With Care Community, you can:
- Centrally identify and manage single or multiple care plans and patient care teams
- Team members can view all tasks, across all patients, on one screen
- Create custom care plans with flexible template options
- Leverage the latest FHIR standards to promote interoperability across the care team
- Notify care team members of assigned tasks, changes to care plans and patient events like admission, discharge, or transfers
- Work within provider point of care workflow with real-time access to patient information and care plans
Patient Engagement Portal: The patient engagement portal provides patents with direct access to their own shared care record. With Personal Community, patients and their designees can:
- Review, download, and manage care records
- Upload data from home or personal medical devices
- Schedule appointments
- Complete forms
- Receive alerts and notifications
- Learn about conditions and adopting healthier behaviours
- Securely interact with care providers
Health Insight: Provides real-time, actionable insights for care, and in-depth retrospective analysis for clinical, business, and population health management. Health Insight allows for the:
- Analyse patient data at a population-level using a relational data model
- Identify patients for community-wide care management, registries, and population health management
- Create clinical and business reports and dashboards
- Highlight gaps in care with event and time-based notifications to clinicians and care managers
- Monitor performance over time for process improvement and quality measurement
