Empowering Change Through Social Prescribing

Susan, Social Prescribing Link Worker: Empowering People to Make Life Changes

Susan’s journey as a Social Prescribing Link Worker is built on a deep passion for helping others improve their lives. After working as a careers advisor on a National Lottery-funded project designed to help people overcome barriers to employment, Susan transitioned into social prescribing when the project came to an end.

In her current role, Susan has found a new way to empower people. “I love helping people take control of their lives and try new things,” she says. “It’s about giving them the opportunities to make positive, lasting changes that truly improve their wellbeing.” This commitment to improving the lives of others is what drives Susan every day in her work.

Case Study: Looking to Reconnect

One of Susan’s recent clients, G, was struggling with low mood due to poor health and the emotional toll of supporting his wife through cancer treatment. He had also been made redundant and found it difficult to connect with others. G enjoys DIY and making things but had trouble finding social outlets.

Susan introduced G to local groups, including the Long Sutton Men’s Shed. G joined the group, paid the annual membership, and is now excited to learn new skills and meet new people. This connection has given him a renewed sense of purpose.

Empowering Change, One Step at a Time

Susan’s work shows how social prescribing can change lives. By listening to people’s needs and connecting them with the right opportunities, Susan helps individuals like G regain purpose and improve their wellbeing. It’s all about helping people make meaningful changes that enhance their quality of life.
