Supporting you

If you or someone you know has been diagnosed with dementia, or mild cognitive impairment, Lincolnshire’s Memory Support Service can offer information and guidance on how to live as well as possible with the diagnosis.

This service can provide information and guidance relating to the diagnosis as well as support your carer and/or loved ones with how they can support you and themselves in the future.

Referrals can be accepted via the following routes. You do not need to be referred by your GP or healthcare service as you can refer yourself. You can:

The Memory Support Service works alongside community mental health teams, memory clinics and partner organisations across the county.

Admiral Nurses

Admiral Nurses are specialist dementia nurses. They support families when times are difficult, by providing:

  • one-to-one support
  • expert guidance
  • practical solutions to help you live more positively with dementia
