NHS Continuing Healthcare Service User Survey
NHS Continuing Healthcare (CHC) is a package of ongoing care that is arranged and funded solely by the NHS for people who are assessed as having a primary health need. CHC can be provided in a variety of settings outside hospital, such as in your own home or in a registered care home.
The CHC Service are seeking the views and experiences of individuals who use or are affected by their service. If you, or someone you care for, receives CHC funding, whether that’s holding a Personal Health Budget or receiving Funded Nursing Care in a registered care home please answer the following questions. Your feedback will ensure CHC continue to improve the service they deliver to the people of Lincolnshire.
Engagement
We want to hear your views and experiences of the Continuing Healthcare Service, also known as All Age Continuing Care and ask that you share your experiences and feedback to help us identify what is working well and understand where you feel improvements can be made for the future. We are working on the principle that if you are affected by our service in Lincolnshire, you are the best person to help design it.
