Population Health Management Virtual Showcase
We held our first Population Health Management virtual showcase event on 24th May 2023 via MS Teams.
Following introductory comments from Matt Gaunt to set the scene for the day, keynote speakers Sunil Hindocha and Simone Yule inspired us to consider the potential of a PHM approach. We then heard about the real-life application of PHM methodology in two different Lincolnshire PCNs as well as its contribution to the Lincolnshire Frailty Programme. Bec Richmond provided an excellent overview of the core principles of PHM, leading into a lively panel Q&A before a well-deserved break for lunch.
Attendees then selected one of five breakout workshops, with covered a wide range of topics from decision quality to international application of PHM.
Thank you to all of those who joined us and made it such an engaging and interesting day! We hope it encouraged you to see the potential of PHM across the whole health and care system.
